My medical record is your personal health record (PHR) provided by University Hospital Southampton NHS Foundation Trust.



Patients at University Hospital Southampton NHS Trust


Anyone who is (or has been) a patient at University Hospital Southampton can sign-up for an account (and then login) using their NHS Login. Alternatively, you can be registered for an account by your clinical team.



Patients at other hospital trusts using My medical record


If your hospital trust is using My medical record for your care, you can log into the app using the same details you use on the web version. 
The app stores and displays personal and medical information, some of which will be loaded from hospital systems, including blood results, clinic letters and outpatient appointment information.
It also allows you to add your own information about your health, including details which may be being monitored as part of your current treatment, such as your weight or blood pressure.
