You can select folders to index in settings. You can choose multiple locations to index from. Sub folders and sub folders of sub folders are indexed. But to be able to use playlist template items in playlists you must have some kind of folder structure or at least one nesting of sub folders in the indexed location.

After indexing you can select which folders should be used as playlist template items. Playlist template items can be arranged in a playlist template, which can then be used to generate playlists from. You can also assign color-tags to library items, which can be used to generate smart playlist templates which emulate the feel of a playlist by choosing random playlist template items and random media items based on the color connections formed from the tags in the playlist template.

To generate a playlist you must first create a playlist template or smart template and populate it with items. Then you can click "generate and save playlist" and choose a location for the outputted M3U playlist file. Which you can then play in VLC, Noob Player or other popular media players.

Indexing a large library can take up to a few minutes.
